What we do
St. Mary’s Health @ Home provides you with the care you need at home. The team consists of Nurses, Personal Support Workers, Social Workers, Occupational Therapists, Physiotherapists, and Rehab assistants. The team works closely with you to ensure your care plan at home meets your needs. Our goal is to make your life at home as easy as possible.
Hospital to Home Program
24 Hours
Visit you within 24 hours upon arrival to your home.
3 Day Check in
Check in with you every day for the first 3 days.
Community
Work with other local community resources.
How the Program Works
Before Discharge
The Hospital @ Home Navigator and team meet with you and your family to create your care plan. The plan is shared with the Home Care team, and your first home visit is scheduled after discharge.
Upon Arrival at Home
On discharge day, you will get a call from a member of the St. Mary’s Health @ Home team to confirm you have arrived home safely and to review your information.
Additional Information
When the Program Begins
The care plan is created while the patient is in the hospital, ensuring readiness as soon as they’re discharged.
If Readmitted to the Hospital
If your medical condition changes and requires re-admission, St. Mary’s Health @ Home will continue supporting you when you return home. The team will help with the transition back home.
Ongoing Care
If you need care after the program, your care team will connect you with the appropriate community organization for continued support. St. Mary’s Health @ Home ensures services are in place before discharging you from the program.
We will assist you in finding a primary care provider if you don’t have one.

Care Services
We are available for individualized services to best suit your needs. Our team can provide Nursing, PSW and Foot Care services in the comfort of your home.
