St. Mary’s Health @ Home is a hospital-to-home program that helps bridge patients as they are discharged from the hospital by providing support to help them transition back to their home.

St. Mary’s Health @ Home ensures that individuals who no longer require hospitalization — but still need medical and personal support — can be discharged safely and continue recovering at home. This approach enhances patient outcomes and reduces hospital re-admissions.
​
St. Mary’s Health @ Home is staffed by an interdisciplinary team including nurses, personal support workers, occupational therapists, physical therapists, and social workers.
​
St. Mary’s Health @ Home provides ongoing, personalized care that supports both clients and their caregivers, helping clients regain independence and continue their healing journey.
​
St. Mary’s Health @ Home is guided by the principles and values that our founders, the Sisters of St. Joseph instilled in our organization: that it is an honour to serve – and in particular to ensure that those most vulnerable have access to compassionate, high-quality care.

St. Mary’s Health @ Home builds on the success of St. Mary General Hospital’s hospital-to-home program, expanding partnerships with local care providers to continue advancing health and social care for people in the community.

St. Mary’s Health @ Home is the first service provided by St. Mary’s Health.
