Advancing Care Beyond Boundaries: Home and Community-Based Care for the Region of Waterloo

Building on a legacy of care, St. Joseph’s Health System launched St. Mary’s Health and St. Mary’s Health @ Home to renew Catholic care in the Region of Waterloo and meet the changing health and social needs of the community.

We ensure a smooth transition home with personalized care, daily check-ins, and flexible support tailored to your needs.

Initial Support

Within the first 24 hours of arriving home, we will visit to ensure you are settling in comfortably. For the first three days, we will check in daily to provide support and address any immediate needs.

Ongoing Care

After the first week, check-ins will be adjusted based on your needs. We’ll also coordinate with the hospital to keep your recovery on track through home visits and phone calls.

Personalized & Community Support

We’ll connect you with local resources like Meals on Wheels and caregiver support. Your care plan will stay flexible, adjusting as needed.
